You will be able to read the IAGenWeb requirements in full as you fill out the form. The ref# before each question refers to the corresponding requirement number on the requirements page.

Note:
In the event that you give your site a "fail" or that we ask you to make some changes, we'll negotiate when that work must be completed. Ordinarily it will need to be done within two weeks of the review period (no later than May 15th).

IAGenWeb occasionally is unable to contact a CC by email. The SC may need to do so if a site appears to have been abandoned. Will you please share your mailing address and phone number? This information is kept in a password protected area of the state site & is never given out to anyone for any purpose other than stated above.
