THE UNITED STATES ARMY, as a Testimonial of Honest and Faithful Service, is herby HONORABLY DISCHARGED from the military service of the UNITED STATES ARMY by reason of    Disqualified at physical examination after acctance for service by local Exemption Board per General Orders No. 11 Headquarters 88th Division Sept. 23, 1917.

in the State of    Iowa When enlisted he was    29 5/12  years of age

and by occupation a   Carpenter

He had  blue  eyes, Light Brown  hair,  Ruddy  complexion, and was    Five   feet  Seven  inches in height.
