THE UNITED STATES ARMY, as a Testimonial of Honest and Faithful Service, is herby HONORABLY DISCHARGED from the military service of the UNITED STATES by reason of     Certificate of Disability -- valvular heart disease per 4th Ind. Hq. 85th Div. 5/18/18

in the State of    Iowa When enlisted he was    23 2/3     years of age

and by occupation a   farmer

He had  Blue  eyes,  dk. brown  hair,     ruddy  complexion, and was       5  feet   7 1/2   inches in height.
