Instructions

All pay allowed under this appointment will be paid by the Medical Department and noted on this letter of appointment.
 
Vouchers for pay will be made out on Form 335. When base pay $50 a month only is allowed the entries on the face of the form will suffice. When additional pay is allowed the whole amount due should be indicated on the face of the form, and the items thereof should be indicated in the second fold of the form thus, e. g.: "Base pay from ----to----," Additional pay in lieu of lodgings and meals, from ----- to-----$------; Additional pay for services beyond seas at----, from---- to----$----; Additional pay as Head Aide from ----to----$----"; Item additional pay in lieu of lodgings and meals must be supported by the responsible Officer's certificate that it was not practicable to lodge her at the hospital. 
 
Vouchers for per deim in lieu of traveling expenses are paid by the quartermaster Corps.

This aide also entitled to additional pay at rate of $62.50 per month, in lieu of meals and lodging, from Aug. 1st, to 15th, 1920, it being impracticable to furnish meals and lodging at this hospital. No quarters being available. 
 

This Aide is not entitled to additional pay in lieu of meals and lodging from Aug. 16th to 19th, 1920 inclusive quarters and lodging having since become available at this hospital.
