This is to Certify, That Bert A. Elledge #2,849-770 Private Co. C. Dev. Bn. #One, 163rd Depot Brigade was discharged from the military service of the United States by reason of Physical Disability at Camp Dodge, Iowa on the sixth of November, 1918; that at the time of his discharge there was erroneously issued to him a discharge certificate  on Form 526, A. G. O. whereas he was entitled to receive a discharge certificate on Form 525, A. G. O., indicating he had been Honorably discharged the service. Having surrendered said certificate so issued him, this certificate is issued in lieu thereof, and by virtue of this certificate he is entitled to be regarded as having been honorably discharged the military service as of the date and place aforementioned.

 

Said Bert A. Elledge was born in Osterbrook, in the State of Iowa. When enlisted he was 22 2/12 years of age By occupation a barber and a farmer. He hade Blue
