THE UNITED STATES ARMY, as a Testimonial of Honest and Faithful Service, is herby HONORABLY DISCHARGED from the military service of the UNITED STATES by reason of    W. D. Telegram dated Washington D. C.  Jan. 18, 1919.

in the State of    Iowa When enlisted he was    21 8/12     years of age

and by occupation a   carpenter

He had  Brown  eyes,     brown  hair,     ruddy  complexion, and was       5   feet     6  inches in height.
