THE UNITED STATES ARMY, as a Testimonial of Honest and Faithful Service, is herby HONORABLY DISCHARGED from the military service of the UNITED STATES by reason of     Physical Deficiency per Surgeon's Certificate of disability and 4th Ind. 163rd Dept Brigade, Camp Dodge, Iowa. Nov. 25, 1918, giving as cause for discharge Valvular heart disease Mitral, insufficiency and Cardiac Hypertrophy. Discharged by order of the Secretary of War, Paragraph 7, General Order.

 

Said    Dale S. Brown was born in    Colesburg
in the State of    Iowa When enlisted he was    21 1/12     years of age

and by occupation a   Farmer

He had  Brown  eyes,     Black  hair,     Ruddy  complexion, and was       5   feet   eleven    inches in height.
