for Old Age Assistance, as provided for by Chapter 266*F1,
Code of Iowa, 1935 as amended by Chapter 139 Acts
of the Forty-seventh General Assembly in Extraordinary Session,
has been approved for a period of two years, and may be
renewed or reinstated from time to time.

The assistance furnished under this certificate and order,
together with subsequent payments made to the beneficiary
named herein, under this order or subsequent orders of the

to whom assistance is hereby granted or by the spouse. |

Under the provisions
of the Act it becomes the duty of the recipient to give
immediate notice to the County Board of Old Age Assistance of
any change in economic condition that would call for a

Continuance of assistance beyond the above mentioned date will
be determined at the end of the period above mentioned.
