       This is to certify that your application for Old Age Assistance, as provided for by Chapter XIX, Acts of the Forty-fifth General Assembly in Extraordinary Session, has been approved in the amount of          $72.00
for the period beginning the first day of    June     , 193 5,        and ending the       30th   day of
November, 1935   ,   to be paid each            month                               in equal installments 
of      Twelve       dollars.
 
       Under the provisions of the Act it becomes the duty of the recipient to give immediate notice to the County Board of Old Age Assistance of any change in economic  condition that would call for a change in the amount of assistance required.

       Continuance of assistance beyond the above mentioned date will be determined at the end of the period above mentioned.

    The granting of the above order constitutes a lien against the real property of the person named in said order and / or against the real property  of the spouse,   David Coonfare  of     RFD #2, Manchester, Iowa   all in accordance with the provisions of section sixteen (16), of Chapter nineteen (19), Acts of the   Forty-fifth General Assembly in Extraordinary Session.
     According to the investigation of the Old Age Assistance Commission, the above named person or persons own and have title to real property located in       Clayton                 County.
