Please Note: The list of Jack’s survivors was different in his obituary and his funeral service notices, both of which I have transcribed below. In researching this, I believe the list of survivors was incomplete in the notification of his death, and the list was complete in the funeral service notice. I have transcribed both articles as written.

Obituary From “Ottumwa Courier,” Friday, November 9, 1945:

Falls Four Floors To His Death

Jack A. Cullinan, 49, of 710 West Main street, was fatally injured at the John Morrell & Co. plant at 9:40 a.m. today when he fell four floors down an elevator shaft in the sausage cooler. He was operator of the elevator and had been a Morrell employe 18 years.

The accident occurred during an inspection of the elevator, which had been raised a few feet about the fourth floor level. A row of sausage racks hanging near the elevator door was struck by an electric lift truck, pushing them ahead. One of the racks struck Cullinan, causing him to lose his balance and fall in the shaft. He died within a few minutes.

Survivors are his wife, Mary E., a daughter, Mary Elizabeth; four sons, Martin Richard, recently discharged from the army and now employed in the hog slaughtering department at Morrell’s; Jack A., Jr., Edward W., and Robert P., all former Morrell employes, now in the service.

Brothers and sisters are Msgr. W.E. Cullinan, pastor of St. Paul’s Catholic church at Burlington; Robert of Indiana, Frank and Thomas, of Des Moines; Mrs. Francis Flahive of Albia and Mrs. Margaret Leads of Chicago.

The body is at the Traul funeral chapel.

“CULLINAN SERVICES,” From “Ottumwa Courier,” Saturday, November 10, 1945:

Funeral services for Jack A. Cullinan, 49, of 710 West Main street, who died Friday morning at the Morrell plant, will be held Monday at 9:30 a.m. at the Traul funeral chapel, and at 9:45 at St. Mary’s Catholic church. A requiem high mass will be celebrated by the Rt. Rev. Msgr. J.M. Walsh, V.F. Burial will be in the Calvary cemetery.

Surviving are his parents, Mr. and Mrs. Pierce Cullinan, Burlington; his wife, Mrs. Mary Cullinan; two daughters, Helen Cullinan and Mary Elizabeth Cullinan; five sons, Jack Cullinan, Robert Cullinan, and Edward Cullinan, of the armed services, Richard Cullinan, recently discharged from the service, and Danny Cullinan; four brothers, the Rt. Rev. Msgr. W.E. Cullinan, Burlington, Frank Cullinan and Thomas Cullinan of Des Moines, and Robert Cullinan of Indianola; and two sisters, Frances Flahive, Albia, and Margaret Leads, Chicago.

Mr. Cullinan was a member of St. Mary’s Catholic church, the Holy Name society, Knights of Columbus council 888, and the Fraternal Order of Eagles, aerie No. 114.

Members of General Shields council No. 888, Knights of Columbus, will meet Sunday at 7:30 p.m. at the hall to go in a body to the Traul funeral chapel and recite the rosary with members of St. Mary’s parish, at 8 p.m.

Pallbearers will be: Thomas Daugherty, John Daugherty, Leo Flahive, James Flahive, Paul Flahive, and Walter Flahive.

The body will remain at the Traul funeral chapel.
