As a Testimonial of Honest and Faithful Service, is hereby Honorably Discharged from the United States Army by reason of: Surgeons Certificate of Disability per 4th Ind Hq 34th Div. Feb. 16, 1918.

Said Alfred H. Wilkinson was born in Oconomonac in the State of Wisconsin. When enlisted he was 24 years of age, and by occupation a Farmer. He had Blue eyes, Dark hair, Fair complexion and was 5 feet 11 inches in height.
Given under my hand at Camp Cody, Deming, N.M. this 19th day of February, one thousand nine hundred and eighteen.
